People living with dementia deserve the highest standards of care and support possible which is exactly what we offer at The Fremantle Trust.

Through our award-winning specialist dementia care services and our highly experienced team of professionals, we aim to support each individual every step of the way; getting to know them, finding out how they wish to live their life, and then developing a unique, personalised care plan for each person in partnership with family, friends and carers.

We also offer palliative care services for those living with dementia in our nursing homes which is focused on improving the person’s quality of life by relieving discomfort caused by a life-changing diagnosis.

Living with dementia

There are 850,000 people living with dementia in the UK today. By 2025, the number is expected to rise to over a million. One reason for this is that we have an ageing population – and dementia is primarily a condition associated with old age.

The term ‘dementia’ describes a syndrome – or condition - can be caused by lots of other diseases. Alzheimer’s disease is the most common cause, but other dementias include vascular dementia, dementia with Lewy bodies and frontotemporal dementia. The main symptoms experienced by a person with dementia are memory loss, confusion and difficulties reacting to everyday changes that happen around them.

What is specialist dementia care?

Dementia care is a specialist type of care service available for individuals living with dementia. It includes practical assistance with day-to-day tasks including preparing meals and getting dressed along with other beneficial activities such as cognitive stimulation and physical exercise.

Exceptional dementia care

At The Fremantle Trust, a person living with dementia will receive support from our specially trained care workers. Our experienced team have developed specialist skills; anticipating a person’s needs, spotting signs of behavioural changes or preventing a build-up of anxieties and frustration. Our environments are different too, specially designed to be enabling, safe, comfortable and enriching and we also offer support plans for every individual designed to capture the uniqueness of each person so that the care provided is genuinely ‘person’ centred.

The Fremantle Trust proudly holds a membership with The National Care Forum (April 2023 – March 2024), an esteemed advocate for excellence in care within the non-profit sector.

Our specialist dementia care team 

High-quality dementia care requires close working across a strong and dedicated team. We ensure all of our colleagues receive comprehensive and robust training using a blended approach and through our in-house Learning and Development team. 

Dementia training starts with an initial induction and progresses regularly using digital learning and face-to-face training, together with coaching in the work setting to ensure that standards are set and continuously improved.

Our care home activities

We offer creative and rewarding activities, as part of group experiences, such as singing, arts and crafts and cognitive stimulation therapy. We also encourage people to try out domestic tasks including baking, or more ambitious projects like putting on a musical! Individual, bespoke activities take place for those who prefer more self-contained pursuits.

Family involvement

We rely greatly on the involvement and cooperation from family and friends of the person who has dementia. They bring a lifetime of knowledge which can aid our work enormously, building up a life story and enriching the person’s experience. Sharing care with families is a simple and effective way in which we can help a person truly ‘live well’ with dementia.
