Everyone Fremantle supports has different needs and wishes so we work with everyone in a different way.

We speak to you and your family or advocate, if you need their help, about all of your likes and dislikes.

We will find out what you can do for yourself.
What you need help to do.
What new things you want to do.

We will ask you about your hobbies and pastimes.
|

|
In fact, we will get to know all about you and your hopes and dreams – we call this a ‘person-centred’ way of planning support.
|

|
Then we will write a person-centred Support Plan with you. This plan will tell the staff what they need to do to support you to live a happy and fulfilled life.

|
You will have a Key Worker - this member of staff who will be your main supporter. This person will get to know you best and will work with you to make sure everything is right for you.
|
|
|
Reviews: You and your key worker will look at your support plan every few weeks to see if it needs to be changed.
No one stays the same all the time and what you need or want can change. Or you may change your mind about something.

Risk Assessment: If you want to try anything new we will work with you to do something called a Risk Assessment. We have to work out if there is anything dangerous about what you want to try and how we can overcome that danger so that you can do the new thing safely.
